APPOINTMENT INFORMATION

- 18 and older and be able to sign your own consents
- Insurance is not accepted
- You can submit your visit invoice to your insurance or HSA account
- Your orders, labs, or local pharmacy prescriptions might be covered by your insurance if accepted
At your visit your provider will:
- Review ICARENP's weight management program and medication options
- Discuss your weight loss goals and prescribe medications if indicated
- Since our providers manage chronic problems we accept patients who might not qualify for other medication programs
Prior to your visit:
- Complete your health forms
Branded medication requests:
- We will send branded medications to your local pharmacy at your request
- Call your local pharmacy and verify your price and coverage for the branded medication
- Verify that your pharmacy has the branded medication in stock
- You will be required to pay for a visit with each branded medication prescription or prior authorization request
- 90-day prescriptions will be prescribed if requested at the time of your visit
