Prior to your visit: 

• Complete your health forms that will be sent to your email. 

Your provider will:

• Review your health forms and reason for seeking our care. 
• We will develop a shared plan of care.
• Send medications if appropriate.

Branded medication requests: 

• We will send branded medications to your local pharmacy at your request.
• Call your local pharmacy and verify your price and coverage for the branded medication.
• Verify that your pharmacy has the branded medication in stock.
• You will be required to pay for a visit with each branded medication prescription or prior authorization request.
• 90-day prescriptions will be prescribed if requested at the time of your visit.
